No, if you have a field mapping in the CRM plugin and you want to either update the field label/name in your CRM, or update the name of the custom field in Outreach, it will not cause any issues and is fine. After you update the field name for a custom field in Outreach, it will also update that anywhere else that custom field shows, and does not affect any syncing or values for that field.
Note, if you are updating and changing the label or name in your CRM, you'll want to do a "force refresh" on the CRM object in Outreach after you make that change so the new label/name is updated in the field mapping.
